NEW ONLINE FORMS AVAILABLE FOR SELECTED PROCEDURES

Graduate Division is moving to online forms for selected procedures starting fall 2020.  Learn how to complete and submit a form using Kuali Build Documentation below.

How do I submit a form?

  1. Open the form for your selected procedure.
  2. Complete all required fields on the form.
  3. Review the form for completeness and correct any error found.
  4. Locate the Form Actions section (right of every form). Click/tap the Submit button to submit your form.

What happens after I submit a form?

  1. After your form submission, you will receive an email notification informing you that Graduate Division has received your form.
  2. Graduate Division will review and validate the data in your form submission.
  3. A request for approval will be automatically sent to your Graduate Chair if needed.
  4. Requests for approval will be automatically sent to committee members if needed.
  5. After all necessary approvals, Graduate Division will perform a final review on the form.
  6. After Graduate Division’s final review, you will receive an email on the status of your form.

NOTE:  Form submission cannot be saved.  If you noticed an error on the form that you submitted, please notify Graduate Student Services at: gdforms@hawaii.edu
