NEW ONLINE FORMS AVAILABLE FOR SELECTED PROCEDURES

Graduate Division is moving to online forms for selected procedures starting fall 2020.  Learn how to complete and submit a form using Kuali Build Documentation below.

How do I submit a form?

  1. Open the form for your selected procedure.
  2. Complete all required fields on the form.
  3. Review the form for completeness and correct any error found.
  4. Locate the Form Actions section (right of every form). Click/tap the Submit button to submit your form.

After Graduate Division reviews and validates the form, you will receive a submission notification email. A submission notification email will not be sent immediately after a form submission. Please notify approvers that the form will be routed through Kuali Build. After further review by your graduate program and Graduate Division, you will receive an email on the status of your form.

NOTE:  Form submission cannot be saved.  If you noticed an error on the form that you submitted, please notify Graduate Student Services at: gdforms@hawaii.edu
