Getting Started

  • You may be required to enter your Graduate Program Chair and graduate program assistant on your form. Consult with your Graduate Program to determine your program’s current Graduate Chair and graduate program assistant who coordinates student records.
  • Before submitting a form, use the UH Faculty/Staff Directory to verify the names and UH email addresses of your graduate program’s graduate chair, graduate program assistant, committee members, or any other UH personnel required on your form.
  • For any non-UH committee members, complete a signed non-UH committee member form before your form submission. The non-UH committee member approval forms are located on the form and also on this page. The non-UH committee member’s signed form must be attached to the form submission.
  • For any errors on the form, please withdraw your submission and submit a new form.
  • Follow up with your approvers if you have any questions regarding the status of your form.
