The following documents must be completed and submitted to the Field Office in order to become a Field Instructor and Field Site.

 

Field Instructor Forms

To be considered for a role as a Field Instructor, you must submit the Field Instructor Non-Compensated Adjunct Faculty Appointment form.  When completing the form, you will be required to submit your most recent resume or CV.  After submission, a representative from the Field Office will call you to verify your information and request some additional material from you.

Once appointed as a Field Instructor, you can complete this form to receive your UH Manoa ID.

Agency Required Materials

Affiliation Agreement

We require an Affiliation Agreement to be executed between the partner organization and our institution.  The University’s Legal Affairs department has created a standard Affiliation Agreement that is used by the Department of Social Work.  Any changes to this document must be first approved by our legal counsel.  A representative from the Field Office will send you a copy of the Affiliation Agreement with instructions to your email.  The document must be signed by the appropriate signing authority at the partner organization.

Field Site Profile

Please complete a Field Site Profile by completing the New Program Form. The Field Site Profile is used to help students learn about your agency and program as well as the type of experiences they could anticipate having.

All students are covered by a professional liability insurance policy.
