For substantial course modifications, follow the same procedures as for course creation. This is if the proposed changes affect the nature and content of the course such that the course essentially looks like a new course.
For minor course modifications, submit a UHM-2 form with a clear justification for the proposed changes and a current syllabus (reflective of term of effect or remove all dates) to UHM Course Actions: firstname.lastname@example.org. Refer to the Submitting Guidelines for UHM Forms for instructions on completing UHM forms.
After review by the Graduate Division, the forms will be returned to the Office of the Vice Provost for Academic Excellence for final approval.
