Annual Graduate Review Checklist
Fill-out and use as the first page for your 2021-22 academic year annual review packet, due to Graduate Instruction Committee Chair Dr. Rob Cowie (cowie@hawaii.edu) by Monday, September 18, 2023. First year students are exempt.
Annual Graduate Committee Meeting Report Form
Submit to the Zoology Graduate Program office annually (after your first year), excluding years when Form II and Form III submitted.
Submit to the Zoology Graduate Chair when completing Form 1.
Doctorate Form 2a – Dissertation Committee Approval
As soon as PhD students form their committees, and before their comprehensive exams, they must fill-out this form and submit it to the Zoology Graduate Chair.
Submit to the Zoology Graduate Chair when completing Form 2 and Form 3.
Submit this form to the Zoology Graduate Chair for sick leave and other personal matters requiring a brief time off. If you wish to take a leave of absence for an entire semester, then you must petition the Graduate Division in advance (see below).
