MPH Program Plan instructions:

1. Click on the Google Sheet link for your specialization. This will take you to a Google Sheets webpage to make a copy of your specialization’s program plan.
2. On the Google Sheets webpage, click on the “Make a copy” button. This will create a Google Sheet in your Google Drive.
3. Rename the document by clicking on the title “Copy of [specialization]: MPH Program Plan (Form 14)” and replace “Copy of” with your Last Name, First Name.
4. Share this document by clicking on File > Share > Share with others. Share this form with the PH 789 instructor, your faculty advisor, and the Graduate Advisor (phgradv@hawaii.edu) by adding their emails in the “Add people, groups, and calendar events” box.  Then click the “Done” button.
