Student Success Center may offer in-person make-up exams for students.

• Please note that the Student Success Center does not host remote online proctoring for make-up exams, only an in-person option.
• Instructors must have offered an initial proctoring date for students.
• If the student is unable to come into SSC for the make-up exam, the instructor will be responsible to host another remote exam

Make-up exam requests must be submitted by the Instructor a minimum of 5 days before the make-up exam date range.

• Due to proctor availability, SSC highly recommends instructors utilize a date range for make-up exams.

Please send the completed make-up exam form to uhmexam@hawaii.edu and your request will be processed in 3-5 business days.
