Make sure to update in our site manuals:
- How to report patient safety events, safety resources and roles
- Quality improvement priorities
- I-PASS and transitions of care policies and procedures
- Information on supervision expectations and progressive autonomy
When you have service line rounds (including group supervision, group didactics, or multi-disciplinary team rounds for FTC; weekly team meetings for KBHS), make sure to regularly ask:
- Any safety events to report and discuss?
- Root cause analyses or other activities that include analysis, as well as formulation and implementation of actions
- Specialty-specific data on quality metrics and benchmarks
- Is there a culture supportive of requesting assistance?
- How is supervision going?
- Does the clinical site/department provide residents/fellows with a supportive culture forrequesting assistance?
- Are supervision policies and procedures followed?
- Are there any patient care vulnerabilities due to workload, and if so, how can we address them?
- Have there been issues with resident/fellow fatigue?
- Collaboratively develop patient care plans with other specialists
When you have walk rounds or other forums where you interface with other medical specialties (e.g., CL service), make sure to:

When you have walk rounds or team rounds (e.g., FTC rounds) where patients/families are part of the meetings, make sure to periodically ask:
- Do you know the names and roles of the attending physicians and residents/fellows taking care of you?
- Do you feel that you have adequate contact with the attending physicians?
- Do you feel that you have adequate contact with the resident/fellow team?
When you are signing in or signing out, make sure to also discuss:
- Signouts
- Transfers
- Strategies for improving transitions of care
