IN OUR SITE MANUALS, MAKE SURE TO UPDATE:

• How to report patient safety events, safety resources and roles
• Quality improvement priorities
• I-PASS and transitions of care policies and procedures
• Information on supervision expectations and progressive autonomy

WHEN YOU HAVE SERVICE LINE ROUNDS (INCLUDING GROUP SUPERVISION, GROUP DIDACTICS, OR MULTI-DISCIPLINARY TEAM ROUNDS FOR FTC; WEEKLY TEAM MEETINGS FOR KBHS), MAKE SURE TO REGULARLY ASK:

• Any safety events to report and discuss?
• Root cause analyses or other activities that include analysis, as well as formulation and implementation of actions
• Specialty-specific data on quality metrics and benchmarks
• Is there a culture supportive of requesting assistance?
• How is supervision going?
• Does the clinical site/department provide residents/fellows with a supportive culture for requesting assistance?
• Are supervision policies and procedures followed?
• Are there any patient care vulnerabilities due to workload, and if so, how can we address them?
• Have there been issues with resident/fellow fatigue?

WHEN YOU HAVE WALK ROUNDS OR OTHER FORUMS WHERE YOU INTERFACE WITH OTHER MEDICAL SPECIALTIES (E.G., CL SERVICE), MAKE SURE TO:

• Collaboratively develop patient care plans with other specialists

WHEN YOU HAVE WALK ROUNDS OR TEAM ROUNDS (E.G., FTC ROUNDS) WHERE PATIENTS/FAMILIES ARE PART OF THE MEETINGS, MAKE SURE TO PERIODICALLY ASK:

• Do you know the names and roles of the attending physicians and residents/fellows taking care of you?
• Do you feel that you have adequate contact with the attending physicians?
• Do you feel that you have adequate contact with the resident/fellow team?
