Consent Form

Overview: A group of researchers from the University of Hawaiʻi 

are conducting a study of community culture, cognition, and attitudes. We are inviting adults age of 18 and above to participate in this study. The research group members are from the William S. Richardson School of Law, College of Social Sciences Department of Psychology, the College of Education, and the John A. Burns School of Medicine Department of Native Hawaiian Health. The principal investigator of this study is Dr. Rebecca Delafield, an Assistant Professor in the Department of Native Hawaiian Health. 

Taking part in this study is your choice.
Your participation in this project is completely voluntary. You may stop participating at any time. If you stop being in the study, there will be no penalty or loss to you. 

What am I being asked to do?
Participation in the project will consist of responding to questionnaire type questions, filling out brief background information about yourself, and pressing keyboard keys in response to a computerized program (not too much different from a computer game). Data from these responses will be summarized into broad categories. We intend to publish the broad results and our analysis in one or more scholarly articles. No personal identifying information will be included with the research results. Completion of this survey will take approximately 10-15 minutes.

What are the risks and benefits of taking part in this study?
The researchers believe there is little or no risk to participating in this research project. You may email the research team at HIstudyemail@gmail.com with any concerns you may have regarding the research. There will be no direct benefit to you for participating in the study. The results of the project may help us better understand community culture and decision-making. 

Confidentiality and Privacy:
Research data will be confidential to the extent allowed by law. We will not ask you for any personal information, such as your name or address. Please do not include any personal information in your survey responses. All research records will be stored in a password protected hard drive in the primary researcher’s office for the duration of the research project. Other agencies that have legal permission have the right to review research records. The University of Hawai’i Human Studies Program has the right to review research records for this study.

Future Research Studies:
Identifiers will be removed from your identifiable private information and after removal of identifiers, the data may be used for future research studies or distributed to another investigator for future research studies and we will not seek further approval from you for these future studies.
