Kūpuna Support Navigator Program

FY23 Community Funded Project

The Kūpuna Support Navigator Program employs Community Health Workers, referred to as Kūpuna Support Navigators, to connect kūpuna (older adults in Hawaiian) with existing community resources by understanding their individual goals and needs. With support from Representative Ed Case’s office under the Consolidated Appropriations Act 2022, the Kūpuna Collective launched the Program to bridge service gaps for vulnerable kūpuna by:

  1. Offering a training program for continuing education and skill enhancement of community health workers and aging network stakeholders.
  2. Implementing coordinated care models that integrate home and community services through Kūpuna Support Navigators
  3. Conducting impact evaluations to refine training, sustain service models, and identify opportunities for scalable workforce development.

The development and implementation of the Kūpuna Support Navigators program were a collaborative effort between the UH Center on Aging and the Hawai’i Public Health Institute. Services are delivered by four distinguished providers: Hawai’i Meals on Wheels, Hui Hau`ula, Na Hoaloha, and Our Kūpuna.
