What: The Kūpuna Support Navigator (KSN) program is an innovative initiative that trains Community Health Workers (CHWs) to serve as navigators for Hawai’i’s older adults (kūpuna).
Why it was created: It addresses the fragmented landscape of health and social services for older adults in Hawai’i, where many face challenges such as chronic diseases, disabilities, language barriers, and financial struggles.
Goal: To build and strengthen the capacity of the aging network by bridging silos between home and community-based services to support vulnerable older adults.

Funding: This initiative is funded through the Consolidated Appropriations Act 2022, supported by Representative Ed Case’s office, and is implemented by the Kūpuna Collective, a network of 270 member organizations dedicated to supporting Hawai’i’s aging population.
