RIO Applications and Forms

RIO Applications for the 2024-25 academic year are now available, see below.

The RIO Application and Change forms are accepted electronically.

Event and Fundraiser Forms must be submitted to Meeting and Events Services in Campus Center 209. If you wish to submit these forms electronically, please send to sld@hawaii.edu and it can be routed to MES.

Application reviews will begin on September 1st. Review and approval of applications takes approximately 10 business days once the application is properly completed and moves to final review. Please plan accordingly to avoid having your organization’s registration lapse or approval delayed.

RIO Registration and Change Forms*

• Authorized Representative Individual Signature Form (RIO Agreement)
• RIO Change of Representative Form
• This form is only for approved RIOs who wish to change a representative, and must be submitted with the RIO Agreement.
• Current representatives who wish to become the new main contact should re-submit the RIO Agreement, not the Change of Representative form.

• Alcohol Waiver Form: Request for Service or Sale of Alcoholic Beverages by or to Students

Additional forms will be provided while reserving space at the Campus Center Complex.
