Applying for UHM’s Performance Certificate

The following materials are required for a complete application to UH Mānoa’s Performance Certificate program.

• Completed Application Form
• Performance Audition or Audition CD/DVD (visit Audition Requirements)
• 1 Letter of Recommendation accessing applicants musical skill, potential, work habits, and any other factors that pertain to the applicant’s ability to attain this certificate.
• Please send original transcripts from institutions you have attended. If the transcripts are in another language, submit a version officially translated into English.
• TOEFL test score of at least 450 (Paper), 39 (IBT Test), or 133 (Computer); or IELTS minimum 4.0
• Application Fee: $200
• Health Forms
• Bank Statement with $40,000 for student VISA proof.
