Health Clearance

Students studying abroad on MIX programs are required to see a physician before departure to obtain a health clearance. Information from the health clearance is confidential and is not used during the application process. Failure to submit a Health Clearance Form prior to program departure will result in the student being withdrawn from their program.

Students may visit their family medical provider, University Health Services, or a local clinic to complete their Health Clearance Form. The MIX Confidential Health History Form must be completed before the MIX Health Clearance Form can be completed. Aim to book your appointment with your healthcare provider 2-3 months before your departure date as appointments may fill up quickly. Completed forms should be submitted to the MIX Office in the original signed/stamped and sealed envelope from your physician. Please do not open it.

Due to the COVID-19 pandemic all MIX programs have been suspended through the end of Spring 2021 [details here]. A decision on Summer 2021 programs will be announced at a later date. To prevent the spread of COVID-19 at UH Mānoa, all MIX Office staff are working remotely. You may continue to book appointments with MIX staff, however, all appointments will be conducted via Zoom until further notice. If you have questions that do not require an appointment, please contact us via email.
