For substantial course modifications, follow the same procedures as for Course Creation (UHM-1 process). This is when proposed changes affect the nature and content of the course such that the course essentially looks like a new course.

For minor course modifications (e.g., update terminology, change to variable credit, remove prerequisite(s), etc.) submit a UHM-2 form with a clear justification for the proposed changes and a current syllabus (reflective of term of effect or remove all dates) to UHM Course Actions: uhmcrse@hawaii.edu by the posted deadlines (Fall: October 15; Spring: March 15) and follow internal college/school review deadlines.  Refer to the Submitting Guidelines for UHM Forms for instructions on completing UHM forms.

If the proposed changes affect program requirements (e.g., modification changes course to be required, change is to retire course and removes a listed elective, etc.), a Program Modification memo with required documents will be required with the UHM-2 form.

After review by the Graduate Division, the forms will be returned to the Office of the Vice Provost for Academic Excellence for final approval.
